I hereby grant permission for the University and the University’s representatives, agents, licensees, sub-licensees, and any entities and successors in title deriving rights from the University (authorised entities) to use materials involving my participation. This permission includes the uninterrupted, irrevocable, time-limited, worldwide use of the materials for any legally permissible purpose across all media now known or hereafter developed, particularly for use in the following fields:

1. Recording (producing copies that may be used for publication);
2. Modification and processing;
3. Digitalisation;
4. Reproduction through printing, recording on compact disc or floppy disk;
5. Storing in computer memory;
6. Producing computer printouts;
7. Marketing;
8. Lending or making reproduced copies available;
9. Uploading, in whole or in part, to an internet network to allow user access and broadcasting;
10. Publishing and disseminating in whole or in part online, including storage in RAM;
11. Recording through printing, computer technology, photographic, and digital methods;
12. Reproduction using printing, computer technology, photographic, and digital methods, regardless of quantity;
13. Use in audiovisual, multimedia, and radio works,
14. Distribution allowing individual access to the work at any chosen time and place;
15. Broadcasting in television programmes and audiovisual works via cable, wireless transmission, ground station, or satellite, and screening in cinemas for public audiences;
16. Simultaneous and integral broadcasting in television programmes and audiovisual works in collaboration with another television organisation;
17. Rental or leasing;
18. Publication in books and other printed materials (e.g., albums, catalogues, lexicons, calendars), as well as multimedia publications, either individually or in collections with works by others;
19. Dissemination in whole or in part for promotional and advertising purposes, including posters, brochures, announcements, advertisements, and audiovisual or multimedia ads, regardless of format.

In the event that any rights arise in the above materials, particularly rights to intangible assets of which I would be the owner, I hereby declare that I transfer these rights to the University free of charge upon their establishment, without any restrictions, including time or territorial limitations. This transfer includes the right to exercise derivative rights, and I authorise the University to exercise author’s moral rights to the extent it chooses. Economic copyrights are transferred to the University in all fields of exploitation listed above. 

I further declare that I release the University, along with its representatives, agents, licensees, sublicensees, and any entities or successors in interest deriving their rights from the University, from any obligation to indemnify against claims, expenses (including attorneys’ fees), or other liabilities arising from the use of the materials. This includes, without limitation, any claims or actions related to defamation, libel, or violation of privacy or publicity rights, whether known or unknown. 

Pursuant to Articles 12 and 13 of the General Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, repealing Directive 95/46/EC (General Data Protection Regulation) (OJ EU.L.2016.119.1), hereinafter referred to as GDPR, and the Act of 10 May 2018 on the protection of personal data (consolidated text: Journal of Laws of 2019.1781 as amended), I acknowledge that the Controller of personal data, including my image (the entity that determines the purposes and methods of personal data processing), is the University of Humanities and Economics in Łódź, ul. Sterlinga 26, 90-212, entered in the register of non-public higher education institutions under number “30”, Tax Identification Number (NIP): 7251014115, and that:

1. The contact details of the Personal Data Protection Officer are the same as those of the University of Humanities and Economics in Łódź, ul. Sterlinga 26, 90-212, entered in the register of non-public higher education institutions under number “30”, Tax Identification Number (NIP): 7251014115, e-mail: iod@ahe.lodz.pl;
2. Personal data is collected and processed as part of activities related to the use of the recorded image by authorised entities in the indicated fields of exploitation;
3. Everyone has the right to request from the Controller access to personal data, as well as their rectification, deletion, restriction of processing, the right to data portability, and the right to object to processing. Everyone has the right to withdraw consent to the processing of personal data that they have given to the Controller. The withdrawal of consent shall not affect the lawfulness of processing carried out before its withdrawal. Everyone has the right to lodge a complaint with the Supervisory Authority, which is the President of the Personal Data Protection Office;
4. Any rights of the data subject can be exercised by making a declaration using the contact details of the Controller or the Personal Data Protection Officer indicated above.
